Plan ahead to make the most of your health care visit

A visit with your health care provider can be overwhelming. There are a few things to consider before, during and after your visit so you make the most of the time you have with your health care provider.

Prepare a discussion checklist.

Your health care provider will ask you several questions. The amount of questions depends on the reason for your visit and whether this provider has seen you before. Your answers help your provider understand your health and your health concerns.

Be prepared to discuss:

  • The main reason for your visit
  • New concerns
  • Past and present medical conditions
  • Any past surgeries
  • Prescription and nonprescription medications, vitamins and herbal remedies
  • All allergies
  • Immunization records
  • Family medical history

Write down your main concerns.

Prepare a list of main concerns. Rank them in order of importance.

You may want to ask:

  • How can I improve my health?
  • Are there conditions or diseases I am at risk for?
  • What are possible side effects of medications I am taking?
  • Are there any available resources that will help me understand my medical conditions?

It also is important to tell your health care provider about anything in your personal life, such as an illness in the family, financial worries or job difficulties, that may affect your health.

Describe new symptoms or recent changes in symptoms.

Your health care provider may ask you detailed questions, such as:.

  • How long have you had the problem?
  • What does it feel like?
  • How often do the symptoms occur?
  • What brings on the symptoms?
  • What reduces the symptoms?
  • Is the problem getting worse?
  • Have other family members had similar problems?

Having a family member or friend with you can be helpful. Having two people hear the discussion can help later when you want to remember what was talked about. In addition, you can read the provider's notes from your visit through the patient portal.

Expect to ask all of your questions and have them answered. Talk with your health care provider about resources that may help answer your questions. This may take more time than this visit allows. If you need more time for questions, you may need to schedule a follow-up visit or a phone call.

Recap what you heard after the visit.

It may be helpful to write down what you learned from your visit. Include information about how your health care provider is planning to help you and what actions you can take to improve your health. Request an after-visit summary that includes visit diagnoses, treatment plans, medication list and patient education, if one was not provided to you.

Make the most of your doctor visit

A visit with your health care provider is a good time to share health concerns and ask questions. Preparing ahead for your appointment can help you get the most from your time together.

When you see your provider, be honest about your symptoms and lifestyle habits . Ask questions to make sure you understand. Taking an active role in your health can help you get the best care possible.

How to Prepare

Before your visit, jot down your questions and concerns. You may want to ask things like:

  • Am I due for any screening tests?
  • Should I keep taking this medicine?
  • What could be causing my symptoms?
  • Do I have other treatment options?
  • Should I worry about my family medical history?

Also be sure to write down all medicines, vitamins, and supplements you take. Include over-the-counter medicines and herbal supplements as well. Bring this list with you to your appointment.

If you are having symptoms, write down details before the visit.

  • Describe your symptoms
  • Describe when and where they appear
  • Explain how long you have had symptoms and if they have changed

Put the notes in your purse or wallet so that you do not forget to bring them. You can also put the notes in your phone or in an email to your provider. Writing things down makes it easier to remember details at the time of your visit.

If you need support, invite a friend or family member to come with you. They can help you understand and remember what you need to do.

Make sure to have your insurance card with you at the time of your visit. Tell the office if your insurance has changed.

What to Share and ask

What you do and how you feel can affect your health. Here are some things you want to share.

Life changes. These may include:

  • Job changes
  • Family changes, such as death, divorce, or adoption
  • Threat or acts of violence
  • Planned trips outside of the country (in case you need shots)
  • New activities or sports

Medical history. Go over any past or current health conditions or surgeries. Tell your provider about any family history of disease.

Allergies. Tell your provider about any past or current allergies or any new allergy symptoms.

Medicines and supplements. Share your list at your appointment. Tell your provider if you are having any side effects from your medicines. Ask about special instructions for the medicines you take:

  • Are there possible interactions or side effects?
  • What is each medicine supposed to do?

Lifestyle habits. Be honest about your habits, your provider will not judge you. Alcohol and drugs can interfere with medicines or cause certain symptoms. Tobacco use puts you at risk for a number of health problems. Your provider needs to know about all of your habits in order to best treat you.

Symptoms. Share your notes about your symptoms. Ask your provider:

  • Which tests might help to find the problem?
  • What are the benefits and risks of tests and treatment options?
  • When should you call your provider if your symptoms do not improve?

Prevention. Ask if there are screening tests or vaccines you should have. Are there any lifestyle changes you should make? What can you expect for results?

Follow-up. Ask your provider when you should schedule more appointments.

After Your Visit

Your provider may want you to:

  • See a specialist
  • Have a test
  • Take a new medicine
  • Schedule more visits

For the best results, follow your provider's instructions. Take medicines as directed, and go to any follow-up appointments.

Write down any new questions about your health, medicines, or treatment. Continue to keep a record of any symptoms and all of your medicines.

When to Call the Doctor

You should contact your provider when:

  • You have side effects from medicines or treatments
  • You have new, unexplained symptoms
  • Your symptoms get worse
  • You are given new prescriptions from another provider
  • You want the results of a test
  • You have questions or concerns

Important information regarding practice closure.

Dr. Cole, Kathie Gonzales, and the staff of St. Petersburg Health & Wellness (SPHW) and Vital Solutions IV Nutrition (VSIVN) have been honored to help care for your health and wellness and provide you a roadmap for your optimal outcomes.

For health reasons, I (Dr. Cole), must follow my own advice and make changes in my lifestyle that support my own wellness. I will be moving into semi-retirement with my focus on authoring educational materials about functional approaches to wellness.  Kathie Gonzales, ARNP, has come to a point in her career where returning to a hospital setting is what calls to her, and is her own best next step professionally.  The result of these changes means that The St Petersburg Health and Wellness office will be closing its doors on 5/28/2022 .   As of that date, Dr. Cole and Kathie Gonzales, ARNP will no longer be available as your personal medical provider. In practical terms, this means they will not be available for appointments, phone calls, or be able to authorize refills for prescriptions. We will make every effort up until that time to provide you with refills for 30-60 days, to give you ample opportunity to obtain a new provider.

If we have been your only medical provider

If we have been your only medical provider, you will need to arrange to have another health care provider take care of your medical needs.  Because our approach is specifically Functional Medicine, you can find additional Functional Medical Practitioners in the area by contacting the American Academy of Anti-Aging Medicine (A4M) at https://www.a4m.com/find-a-doctor.html or the Institute for Functional Medicine (IFM) at https://www.ifm.org/find-a-practitioner/ . Your insurance provider’s directory or local hospital’s physician referral service is another place to check.

Medical Records

Since the virtual business entity of St. Petersburg Health and Wellness itself will continue on, we will be maintaining ownership of all medical records. Legally, all requests for copies of medical records must have a signed authorization to be released. If your doctor needs your medical records, you can sign your doctor’s records request form and they will request them for you. For all other records requests, email your request to  [email protected]  and we will send you the proper medical records release form and any other information to expedite the process. Please understand that the medical record’s email is ONLY for submitting a request for medical records and does not have the ability to answer questions unrelated to the transfer of said records.

For all patients during this transition and beyond:

Continue current protocols.

Continue on your diets and pharmaceutical-grade supplement brands and dosing we have recommended to you. You will still be able to get them through the same portals – Wellevate and WholeScripts – through which you have previously obtained them. Do this in order to continue your “Live Well” lifestyle. This will ultimately keep you healthy and in less need of “sick” (medical) care in the future.

Gastrointestinal Protocols

If you have been on GI supplements to heal your Intestinal Permeability (Leaky Gut), GI tract, and/or Imbalanced Microbiome, continue your specified diet and all of your GI supplements for 3-6 months from the date you started. Again, you can use the same portals to reorder supplements. My rule of thumb for deciding 3 months versus 6 months is this:

  • If your symptoms clear in the 1st month of starting your GI supplements it is generally safe to stop them after 3 months of taking them.
  • If your symptoms don’t clear in the 1st month, then you generally want to stay on them for 6 months.

This guideline generally prevents the symptoms and the underlying condition from recurring.

Hormonal Health

For your hormones or other medications, you will need to find another practitioner to prescribe them for you. In determining which practitioner to choose for this, remember your hormones should be optimized for you because all your hormones are intended to work together as a symphony.

Most healthcare visits are categorized under diagnostic care; when you have a health problem, you visit a healthcare provider who assesses your symptoms, diagnoses your condition, and prescribes a treatment plan.

A wellness visit on the other hand is a preventative healthcare measure. The aim of preventative healthcare is to help you maintain good health and prevent health problems before they develop. The goal is to help you live a longer, healthier life.

Wellness visits assess your lifestyle, evaluate health risks, and screen for health conditions, in order to prevent health problems or catch them in the early stages. Instead of waiting to see a healthcare provider once you have a health problem, the idea is to be proactive about your health and work with your healthcare provider to prevent health problems.

People tend to think that it’s fine to skip their annual wellness visit if they’re feeling healthy. However, a 2021 study notes that wellness visits can play a role in catching chronic health conditions early, as well as helping people control for risk factors that could cause them to develop health issues down the line.

  • Health advice: Your healthcare provider may advise you on steps you can take to improve your health, control risk factors, and prevent disease and disability. This may include nutrition counseling, an exercise plan, flu shot and vaccination recommendations, and fall prevention strategies, among other things.
  • Screenings: Your healthcare provider may recommend that you get screened for certain health conditions such as depression , cholesterol, blood pressure, diabetes, cancer, heart disease, or liver conditions. This may involve blood work, imaging scans, or other screening tests. 
  • Medication review: Your healthcare provider may review your medication and adjust it, if required. This can include prescription medication, over-the-counter medication, vitamins, supplements, and herbal or traditional medication.

These are some steps that can help you prepare for a wellness visit:

  • Fill out any required questionnaires: Your healthcare provider may ask you to fill out a questionnaire before your visit. The questionnaire may include some of the factors listed above. Make sure you do it before your visit, so that you can make the most of your time with your healthcare provider.
  • Carry your medications: If possible, try to carry your medications with you to show them to your healthcare provider.
  • Take your medical documents along: It can be helpful to carry your prescriptions, immunization records, as well as the results of any medical tests or screenings you have had, to help give your healthcare provider a more accurate picture of your health status.
  • Ask someone to go with you: You may want to take a trusted friend or family member along with you for the wellness visit. They can assist you if required, take notes for you, ask questions, and help you remember your healthcare provider’s instructions.
